<p>Care at Home’s Pediatric division has been caring for San Francisco Bay Area kids in their homes for almost fifteen years.  Our specialized pediatric nurses and therapists focus on providing excellent short-term and long-term clinical care for children. Our clinical case managers work with doctors and hospital discharge planners to ensure children transition effectively back home to maximize their stabilization and recovery and minimize the chances of re-hospitalization.  Our long-term skilled nurses care for chronically fragile children and are trained to manage the technologically advanced equipment they may need to manage respiratory and feeding needs and remain safely stable at home.</p>
